Absence Form Use this form to notify the Associate Dean for Student Services of an absence.
Add Form (PDF) Required for adding Directed Study and other restricted law courses. In most cases, professor signature is not required. Submit completed form to the Law School Registrar.
Audit Approval Form (PDF) Required for law students to audit law courses. Professor signature required. Audit credits are included in your tuition bill and count against the maximum number of credits that law students are allowed to register for per semester. If maximum number of credits allowed are exceeded, students must also complete and submit a Credit Approval Form to increase credit maximum.
Change Address & Phone Law Students are responsible for providing the University with current and personal contact information.
Change of Status Form Required for changing Law Student status from Full Time to Part Time, or Part Time to Full Time. Submitted to Associate Dean for Student Services; then to the Registrar for action.
Course Approval Form For those Full Time students wishing to enroll in a Part Time (evening) course or those Part Time students wishing to enroll in a Full Time (day) course. Webform is submitted to Associate Dean for Student Services for approval; then to the Registrar for action.
Credit Load Request Form Approval required for enrolling in fewer than 12 credits (full-time) or fewer than 9 credits (part-time); for Full Time students wishing to enroll in a Part Time (evening) course or Part Time students wishing to enroll in a Full Time (day) course; or for limited approval to increase semester max credits above 17 for non-JD credits. Webform is submitted to Associate Dean for Student Services for approval; then to the Registrar for action.
Degree Application Form (PDF) | Hawaiian (PDF) Required for all graduates to receive a diploma. Hawaiian language diploma is optional. $30 fee required for each diploma. Application must be completed no later than 2 weeks after instruction begins during the semester of graduation and no later than June 1st for the Summer Session.
Directed Study Pre-Approval Form Students must complete this form before submitting the Directed Study Faculty Approval Form.
Directed Study Faculty Approval Form (PDF) Required for adding Directed Study. Students must complete the Directed Study Pre-Approval Form before submitting this form. Supervising professor must complete and sign bottom section of form. Students must also submit a completed Add Form. Supervising professor signature not required on Add Form. Submit completed Directed Study Approval Form and Add Form to the Law School Registrar.
Drop Form (PDF) Required for dropping any law course with a 'W' during semester 'withdrawal w/ permission' period.
Dual Degree Notification Form Required for Law students who are pursuing a dual degree. Students must complete this form within 2 weeks of acceptance to the other degree program.
Graduate Level Course Approval Form Required for any law student requesting to receive JD credit for graduate coursework (600 or higher) taken at UH Mānoa. See Academic Regulations for exceptions. Must be submitted at least 2 weeks before registering for the upper campus course. Must be taken for letter grade, but will be entered as CR/NC in Law School Records, and will not count towards calculation of GPA. Upon successful completion of the course (receipt of a grade of ‘C’ or better), the Law School Registrar will transfer the credits and count them towards the completion of the student’s law degree. The webform is submitted to the Associate Dean for Student Services for approval; then to the Registrar for Action.
Information Request The Information Request Form is for use by current Law students only. This form is required for requesting letters of good standing and enrollment verification. Current Law students who are applying to other degree programs at UH Mānoa may also use this form to request copies of undergraduate transcripts on file with the Law School to be sent to the other UH degree program. Form is sent to the Registrar for processing.
Leave of Absence Application (PDF) Required for any Fall or Spring Term for which a student does not wish to enroll in classes. Submit to Associate Dean for Student Services for approval.
Non-Law Student Data Form Required for non-law graduate students requesting to enroll in a law school course.
Official Transcript Request Form Required to order official transcripts.
Prerequisite Error Form Fill out this form f you are are receiving a prerequisite error when registering for LAW 590G: Estate Planning Workshop and/or LWPA 582F: Emerging Hawai'i Water Issues.
Professional Auditors Application Form Required for non-law professional auditors who wish to take Law classes.
Summer School Approval Form Required to transfer credits from summer programs at other ABA-accredited law schools. Submit completed form to Associate Dean for Student Services for approval. Approval of this form is required before meeting with the financial aid director about summer school financial aid. A separate application process is required if requesting financial aid for attendance at another institution.
Take Home Exam Form Use this web form if you have a take- home final exam for a law course and you need to schedule a release time for your exam. Students must submit this form at least two business days in advance of your scheduled release time.
Visiting Another Law School Approval Form Required for Law Students who wish to visit another law school. This form must be submitted and approved prior to commencing studies at the other law school.
